II. PROCESSING OF APPLICATIONS FOR LEAVE

A. VACATION LEAVE

1.Secure Leave Form from Personnel Section 2 min. Verbal Request

2.accomplish properly the Leave Form 2 min. Leave Form

III. PROCESSING OF MONETIZATION OF LEAVE CREDITS

3. Accomplish the Leave Form and return the same to personnel in-charge for computation. (A request to monetize shall be attached to the leave form if sick leave credits are used.)
